Course Launch Announcement: Preparing for MACRA – Achieving Quality in the Management of Heart Failure

Target audience: The intended audience for the activity is Primary Care Physicians, Hospitalists, Cardiologists, Quality Improvement Professionals, Pharmacists and Care Coordinators.

On Friday, October 14, 2016, the Department of Health and Human Services announced the final rule for the Quality Payment Program, part of the Medicare Access and CHIP Reauthorization Act of 2015 (MACRA). This program is “focused on moving the payment system to reward high-value, patient-centered care.”
This educational activity is designed to help individual practitioners and organizations prepare for the quality component of both pathways of the program–the Advanced Alternative Payment Models (APMs) and The Merit-based Incentive Payment System (MIPS)–as it relates to the management of heart failure (HF) patients. You will be introduced to Central Med Health System, a “virtual” health system in the Southwest initiating a Quality Improvement project in heart failure. You’ll meet many members of their team and have the opportunity to guide them as they engage in their QI initiative.
