Are You Ready for MACRA: What are the Factors that Influence Heart Failure Readmissions?

In 2016, the Department of Health and Human Services announced the final rule for the Quality Payment Program, part of the Medicare Access and CHIP Reauthorization Act of 2015 (MACRA). This program is “focused on moving the payment system to reward high-value, patient-centered care.” In response, RealCME has developed an online virtual Quality Improvement initiative designed to help individual practitioners and organizations prepare for the quality component of both pathways of the program–the Advanced Alternative Payment Models (APMs) and The Merit-based Incentive Payment System (MIPS)–as it relates to the management of heart failure (HF) patients. In this unique and engaging online initiative, you will be introduced to a “virtual” health system initiating a Quality Improvement project in heart failure. You’ll meet many members of their team and have the opportunity to guide them as they engage in their QI initiative. In the first activity of this Quality Improvement (QI) program, we introduced the Central Med Health System (“Central”), a virtual health system in the Southwest that includes a regional 450-bed, tertiary care hospital along with 20 outpatient facilities. In preparation for the quality components of MACRA, Central has established a goal to reduce 30-day readmissions for heart failure (HF) by 50% in two years.
